UNICARE has a new convenient method for obtaining your Prescription needs with Mail Service

September 4, 2004 -- The program covers drugs which by the federal law require a doctor’s prescription and is covered by your medical plan. Check the drug formulary to see which drugs are available through the mail service. Unicare sends you a Certificate of Coverage that details your copayment, maximum days supply, deductibles and specific drug coverage and exclusions. Note: Insulin and Insulin syringes do require a prescription when ordered through the Unicare Mail Service Program.

You are eligible to enroll in the home delivery program if you and your family are currently covered by the outpatient prescription drug program through the pharmacy. When you order the first time you will need to include the form for enrollment of new participants and confidential patient profile forms. In the future you will not need that form unless there are changes to be reported. You must mail the form with the original prescription and the copayment in the pre-addressed postage paid envelope provided. Be sure to write your identification on the back of the prescription.

Then check your mailbox, because your medications will be delivered postage paid directly to your home. If you do not receive your medication in 14 days, please contact the Unicare Medical Mail Service program toll free at 1-866-274-6826. When you are placing your first order be sure to allow a few extra days for it to arive. Call for any further information toll free 1-866-526-9669. While you are there ask about Health Insurance, Life Insurance, or Medicare Supplements.
